Approved Drugs to prevent hair loss

There are only two drug treatments approved by the U.S. Food and Drug Administration (FDA) for male pattern baldness. These are minoxidil (Rogaine/Regaine) and finasteride(Propecia).
Finasteride is recommended as first-line treatment for male pattern baldness, however, if further regrowth is desired after 12 months, finasteride and minoxidil may be used in combination.
Minoxidil
Minoxidil (Rogaine) is a vasodilator originally used as an oral drug (Loniten) to treat high blood pressure. However, minoxidil was discovered to have the side effect of hair growth and reversing baldness. Consequently, in the 1980s, Upjohn Corporation received FDA approval to market a topical solution that contained 2% minoxidil to be used to treat baldness and hair loss as Rogaine, marketed as Regaine outside the USA.
Objective evidence shows that minoxidil is effective for treatment of male-pattern hair loss in both the frontal areas and the vertex area of the scalp. At the conclusion of a 48 week study, improvements were seen in the vertex area regions of 51% of men using 5% minoxidil, 42% using 2% minoxidil, and 13% of placebo users. Among these men, moderate to great increases in hair growth were seen in the frontal scalp regions of 19% of men using 5% minoxidil, 10% using 2% minoxidil, and 3% of placebo users.
The method of action for minoxidil is not known, but many speculate that by widening blood vessels and opening potassium channels, it allows more oxygen, blood and nutrients to the follicle. This can also cause follicles in the telogen phase to shed, usually soon to be replaced by new, thicker hairs. As this effect is temporary and does not seem to change the follicle in any other way, minoxidil needs to be applied regularly (once or twice daily) for hair gained to be maintained.
Finasteride
Finasteride (Propecia), initially marketed as the brand-name drugs Propecia and Proscar by Merck, is an aza-steroid drug. Finasteride is aninhibitor of 5 alpha-reductase, and was originally approved by the US FDA for the treatment of benign prostatic hyperplasia (BPH). The drug works by binding to 5-alpha-reductase, the enzyme responsible for the conversion of free testosterone to DHT.
Merck sought to find the smallest effective quantity of finasteride and test its long-term effects on 1,553 men between ages 18 and 41 with mildly to moderately thinning hair. Based on their research, 1 mg daily was selected, and after two years of daily treatment, over 83% of the 1,553 men experiencing male hair loss had actually maintained or increased their hair count from baseline. Visual assessments concluded that over 80% had improved appearances.
In 1997, finasteride was approved by the US FDA for the treatment of male pattern baldness. A 5-year study revealed that 9 of 10 men taking finasteride (1 mg daily) experienced visible results (42% of men taking Propecia experienced no further hair loss while 48% experienced no further hair loss and hair regrowth). In clinical studies, finasteride, like minoxidil, was shown to work on both the crown area and thehairline area, but is most successful in the crown area.

Of greatest concern in past and present ages is male hair loss. Males usually develop a receding hairline, which develops into the typical ‘M’ shape as they begin to lose hair from the lateral sides of the forehead. At the same time, a progressive bald patch develops on the crown on the head. When these two areas join, they leave the remaining hair in a horse shoe-shaped area.

Male hair loss occurs when the hair follicles shrink and produce increasingly finer hair. Finally, they yield no hair at all any longer. The causes can range from genetic predisposition to changes in the levels of testosterone, the male sex hormone. Though it is also marginally present in females, testosterone determines the physical differences between men and women – including the degree of ‘hairiness’. For instance, an excess of it can cause an abnormality termed ‘hirsutism’, or abnormally prolific hair growth.

Close to 65% of all males suffer from hair loss to some degree or the other. For those who do, male hair loss may have more serious ramifications than merely cosmetic ones – a recent study conducted by Harvard Medical School linked it to a higher incidence of heart disease. The study proposed that male hair loss on the front of the head indicates a 9% increase in such a risk, while hair loss on the crown indicates a risk factor of as high as23%.

This ominous aspect aside, the options for treating male pattern are presently limited to the use of oral finasteride, topical application of minoxidil, natural hair transplants (artificial fibers being banned by the FDA due to high rejection rate). Of course, camouflage via hair weaving, wigs and toupees is an alternative – probably the most cost-effective and least risky one.

Male hair loss is not a problem that can be taken lightly and the medical community certainly been hard at work isolating cures and treatment modules for it.

Propecia does work in reducing male hair loss and even stimulating marginal hair re-growth. It is certainly useful in long-term maintenance in men (Propecia inhibits the systemic creation of DHT in men only). Minoxidil is more effective growing back hair. However, the benefits are strictly short-term and almost immediately reversed at cessation of treatment.

Dutasteride is a recently developed compound in the battle against hair loss. Controlled studies indicate that its ability to stop male hair loss surpass that of Propecia. This is a very new treatment module and studies on its effects are still incomplete. We do know that it can cause allergic reaction in some users.

The only other clinically established and therapeutically useful formulation is tricomin, which does regenerate hair by kick-starting existing hair follicles back into productivity. It does not have many unwanted side effects and it works even betters when used together with other compounds such as Minoxidil and Propecia.

Hair Transplant is a simple surgical procedure which is done by removing a strip of hair bearing scalp from the back or sides of the head where hair growth is permanent and not affected by Dihydrotestestosterone (DHT). A doctor or medical staff member then divides the hair bearing scalp into grafts. These grafts can vary in size but include: follicular (most common) micro, mini and sometimes even maxi size grafts.
